Replace A Lost or Damaged Credential (Degree, Diploma or Certificate)
For a graduate who needs to replace a lost or damaged credential (Degree, Diploma, or Certificate), the Registrar’s Office offers a credential replacement service.
The cost of a replacement credential is $40.00. Courier service is available within Canada and internationally at an extra cost.
How To Submit A Credential Replacement Request
Requests for a replacement credential can be made by completing the form below. Please note that this form requires a signature and can be submitted by email, fax, mail, or in person at Student Services on any campus. Payment must be received before request will be processed.
